Unfortunately, she has been severely abused and drugged for too long — despite Rambo’s desperate attempts to save her, Gabriela dies in his arms on their way home.
Devastated and enraged, Rambo’s grief turns into vengeance. He returns to his ranch and begins transforming it into a fortress of traps and tunnels — a lethal maze built beneath his land. When the Martinez cartel sends an army of hitmen to kill him in retaliation, Rambo prepares for war.

In the brutal final act, Rambo lures the cartel soldiers into his underground tunnels, using his survivalist skills and combat experience to eliminate them one by one with ingenious and gruesome traps. Explosions, spears, and knives turn the tunnels into a battlefield reminiscent of his days in Vietnam.
Finally, Rambo faces Victor Martinez, the man responsible for Gabriela’s death. In a chilling act of vengeance, Rambo shoots him with arrows, pins him to a barn wall, and cuts out his heart — symbolically avenging Gabriela and ending his long cycle of violence.

After the carnage, Rambo, gravely wounded, sits on the porch of his ranch, reflecting on his life. He remembers all the people he has lost and those he loved. The film closes with Rambo riding away on horseback, bleeding but alive — a warrior who will never truly find peace but will fight until the end.
